I'm sure I'll be corrected, but I feel that the inflated costs of medical care come from the uninsured and under-served regions using the ER as their primary care doctor. Costs rise on the hospital, causing them to increase their charges. It's just a vicious circle.
It's really noticeable when a routine outpatient surgery is billed to Blue Cross at $24,000, and BCBS replies back: "We're paying 4k, the patient pays $100 deductable, go pound sand for the other $19,900."
