My dad told me that story about 8-10 years ago. We’re always at risk of this happening because people are dumb. Smart people can be the dumbest of them all.

Not so fast

That would indicate the presumed problem did not involve the Falcon 9. But the Wall Street Journal quoted unnamed sources Monday saying "the secret payload ... is believed to have plummeted back into the atmosphere ... because it didn't separate as planned from the upper stage of the rocket."

Reuters also reported the satellite "failed to separate from the second stage of the Falcon 9 rocket and is assumed to have broken up or plunged into the sea."

I wouldn't blame SpaceX if Northrop Grumman's payload failed to activate and separate properly.

I would blame SpaceX if it didn't hit the required elevation, or if something on SpaceX's end caused the payload to not activate.

It's going to come down to telemetry data of the rocket and any phone home signal the payload had. And given the secretiveness of the payload, we might never find out.

It's worth noting that normally Spacex supplies the adapter between the second stage and the payload. But, in this case, it was supplied by Northop Grumman. My money would be on Northrop Grumman being at fault.
