Yeah, but mainly because it's inaccessible rather than because of the nature of the airport itself.

Fair point. I don't see why they can't extend the subway another mile over the Triborough to get there - it'd save a ton of headaches. The terminals themselves are just weird, but not terrible.

Because no matter where your train to LGA is going to originate, whether you're extending the N across from Astoria, building another AirTrain off the 7 (like the train to JFK is), or whatever else, you're going almost entirely through residential neighborhoods to do it, which means disrupting people's lives during construction and local opposition from people who don't want the added noise and crowding. If you care enough to read more about it, there was a pretty good discussion about this issue on NYC subway blog Second Avenue Sagas a few months ago.
