Has anyone ever seen a delay of game penalty result in a penalty shot before?

In a game I was watching several years ago the goalie deliberately dis-lodged the goal on a breakaway...delay of game resulted as a penalty shot...it was a D1 game but I do not remember the teams. (Goalie made the stop)....guess that was a good penalty!!
