College Hockey 101: Line changes on the fly

Trying to follow the puck
On a line change while the puck is in play, how do players on the ice/bench know when to come off/take the ice?

Guys know when to change based on how long they've been on the ice, mostly. Depending on the time of the game a shift might be anywhere from :30 seconds to maybe a 1:00 or 1:15. Ideally, if changing on the fly is going to happen, players will change when they won't leave their team vulnerable defensively. It requires good ice vision and a sense of how the play is continuing to unfold. Those on the bench waiting to go out simply know who they are replacing. If it is going to be a non-standard change (coach looking for a particular match-up, special team situation, etc) then one of the coaches will have communicated to the proper player who is going next, who is replacing who, etc.
